Every patient has rights and responsibilities. They apply to all of our patients without regard to gender, sexual orientation, culture, education, economics, religion, language, age, race, color, ancestry, national origin, presence of mental or physical disability, or the source of payment for your care. Patient Privacy

Natividad adheres to Federal HIPAA privacy regulations to insure the privacy of our client’s health information. We have established safeguards to protect the confidentiality of medical information.

Medical Records

You have the right to see your medical record. Patients or their legal representatives may request access to their medical records through the Health Information Management Department.

Ethical Questions & Concerns

If you have questions or concerns about treatment choices or other issues that affect decision-making about a patient’s care, the hospital’s Bioethics Committee is available to consult with patients, their loved ones, and the health care team. If you would like to request consultation by the Bioethics Committee, please contact your physician, nurse, case manager, chaplain, or any member of your care team. All consultations are advisory in nature.

Open Payments Database Notice

The Open Payments Search Tool is used to search payments made by drug and medical device companies to physicians, physician assistants, advanced practice nurses and teaching hospitals. The purpose of the program is to provide the public with a more transparent health care system. All information available on the Open Payments database is open to personal interpretation and if there are questions about what the data means, you should speak directly to the health care provider for a better understanding.
