Natividad is now scheduling appointments for the COVID-19 vaccine for people living or working in Monterey County who are 75 and older or who work in healthcare (Phase 1a, tiers 1, 2, 3). At the appointment, you must provide proof of age over 75 years, or, if a healthcare worker, proof of healthcare employment, as well as proof that you live or work in Monterey County.

Current vaccination tier:

  • Health care workers, long-term care residents and emergency services personnel
  • Residents 65 years of age and older
  • All residents who are at risk of exposure while working in food, agriculture, child care, education, and emergency services
  • People who live with those who are eligible in the following ZIP codes: 93450, 93926, 93927, 93930, 93932, 93954, 93960, 95039, 93901, 93905, 93906, 93907, 93925, 93933, 93955, 95004, 95012. Every person must have an appointment.
  • Individuals 16 to 64 years of age with a medical condition or disability that puts them at a high risk for morbidity and mortality as defined by the California Department of Public Health

After you make an appointment, you will receive a confirmation email with:

Date and time of appointment

Location – Natividad Medical Center; Building 400 Suite 101

Reminder to bring government-issued ID and proof of date of birth

Note: if you do not receive a confirmation email shortly after making an appointment, your appointment has not been scheduled.

Accepted Documentation

Accepted documentation includes one of the following for age:

Driver’s license

California ID card or REAL ID card (from DMV)

Military ID

Passport

Accepted documentation includes one of the following for proof of healthcare employment

Identification badge

Pay stub

W2 or other official document with your name and employer

What to expect at your appointment

You will visit 4 stations:

  1. Check-In: Show ID and proof of age or employment
  2. Vaccination: Review history of allergic reactions, if any, receive information on any mild side effects, get vaccinated
  3. Observation: Remain in waiting area for observation for 15 minutes; 30 minutes if history of allergic reactions

Appointments fill quickly as demand is high.  We will continue to schedule clinics as vaccine becomes available, so please check back.


Based on California Department of Public Health guidance and local considerations, at this time the following individuals are eligible to receive COVID-19 vaccine:

  • Health care workers, long-term care residents and emergency services personnel
  • Residents 65 years of age and older
  • All residents who are at risk of exposure while working in food agriculture, child care, education, and emergency services

Please bring an official form of identification to your appointment that shows your name and date of birth.

Vaccination appointments are for people who live or work in Monterey County.
