The first step to becoming a patient is answering our screening questions to determine if you are a candidate for weight loss surgery.

One you have answered our screening questions, you’ll want to attend an information seminar. Our seminars are held on the third Thursday of each month in the Natividad Conference Room (1st floor behind the Gift Shop). The seminar runs from 5:30 pm to 7:00 pm. Everyone who eventually has surgery must attend one information seminar before their procedure. Make sure to sign in so we can mark that item off your pre-surgical checklist.
Your next step will be to discuss weight loss surgery with your primary care doctor. Your doctor will fax a referral to our office, and we will call you to set up your New Patient Consultation.

New Patient Consultation
On the Natividad campus, follow the signs to Outpatient Surgery on the second floor. Sign in at the registration desk, or call our office directly (831) 755-5555 to let us know you are in the waiting room.
During this visit you will meet with the bariatric surgeon and decide if weight loss surgery is something that would work well for you. Complete the Health History Questionnaire and email it to our office, or print it out and bring it with you to your new patient consultation.
