The first step to becoming a patient is answering our screening questions to determine if you are a candidate for weight loss surgery.

Your next step will be to discuss weight loss surgery with your primary care doctor. Your doctor will fax a referral to our office, and we will call you to set up your New Patient Consultation.

New Patient Consultation

On the Natividad campus, the Bariatric Surgery Clinic for Weight Loss is
located in Building 400 on the first floor of the Barbara Shipnuck Professional Center, in suite 101. Use Entrance 4.  See our map here > 

During this visit, you will learn more about the process, meet with the bariatric surgeon and decide if weight loss surgery is right for you. Complete the Health History Questionnaire and email it to our office, or print it out and bring it with you to your new patient consultation.
