When you are ready to be discharged, your nurse will make sure you have everything you need for continued healing. You will receive a copy of the instructions you need to follow after discharge. You will also get information on any medications you will be taking at home. Please call your doctor or the nursing staff if you have any questions about your instructions.
Discharge Planning
When you are ready to leave the hospital, you will be discharged. When you are discharged, we recommend you have a family member or friend pick you up. Your ride may park temporarily at the front entrance of the hospital. They may not leave the car parked there for more than 20 minutes, and they may not park in the red zone.
If needed, a case manager will help you in arranging the care you need when you go home. This care may include:
- Home care agencies
- Skilled nursing facilities
- Equipment rental businesses
- Visits by a public health nurse
